Please could you answer the following questions and email as an attachment back to me.  You are welcome to feedback by answering briefly or expanding your answers as you wish.  Also, if you need me to elaborate on any issue, just let me know.  This questionnaire is to get a feel for what you and your children think about Excel’s service, and completely confidential.  We’ll publish the results (warts n’ all!) on the Excel website when it’s updated for 2011.

Q1.  Do you feel that you and your child(ren) have been made welcome and made to feel valued as an individual when coming to camp?

Q2.  Are you and your children happy with the environment at camp and the range of sports activities and equipment used at camp?

Q3.  Did you feel that your child(ren) are nurtured in their wellbeing in their experience at camp?

Q4.  Did you feel your child(ren) are supported by staff in their sports sessions?

Q5.  Did your child(ren) feel that there was a wide enough range of sports and activities to engage in at camp?

Q6.  Did you and your child(ren) feel there is sufficient mutual respect, trust and open communication with Excel and the staff at camp?

Q7.  Are you confident that Excel will treat your child(ren) equally and fairly at camp?

Q8.  Are you confident that Excel evaluates and improves its programme based on the feedback from you and your child(ren)?

Q9.  Are you confident that Excel is as well managed service?
